Payment Options

Chirn Park Health Group provides HICAPS facility to provide on the spot Private Health Fund extras rebates so you then only need to pay the Gap. You may make payments via Cash, Mastercard or Visa as well as Eftpos.

We also provide Medicare Easyclaim for CDM Programs when patients are referred by their GP on this program.

Appointments

Consultations are by appointment only. You may contact us via email or call for an appointment.

Upon making an initial booking, you will be sent via text an online initial patient consultation form that we request you fill in prior to your appointment. Alternatively, we kindly request that on initial treatments, you arrive 15 minutes earlier to fill in our new patient questionnaire form so we can therefore maximise your treatment time during your consultation.

We will also send you a text as a reminder of your appointments. This will have a Covid questionnaire that we kindly request you fill prior to each appointment.

Do I need a Referral?

You generally do not require a GP Referral to see our Practitioners.

Referrals are required for:

  • Veteran Affairs patients for Osteopaths
  • Medicare /CDM (Chronic Disease Management) consultations. This is at the discretion of your GP. Please advise at the time of booking if you have been referred on this program.
  • CDM visits are NOT Bulk Billed, Medicare rebates do apply.

Non Attendance Policy

Our Clinic calls to remind our patients of their pending appointment. If an appointment is missed without giving 24 hours notice, a cancellation fee may be charged.
